Ridgeview Health Center is looking for a CNPS Full Time Monday-Friday 11a-730pm
GENERAL SUMMARY:
The Clinical Nurse Program Supervisor is on the forefront of quality improvement efforts. Responsible for collecting, tracking, trending and monitoring data reviewing outcomes, compliance and identifying opportunities for improvement. Uses critical thinking skills to present findings and make recommendations to interdisciplinary health care team. Serves as clinical case manager and conducts new admission assessments as census dictates. Completes a clinical record review for current and new admissions.
As a Clinical Nurse Program Supervisor, you can promote and facilitate change. You utilize evidence-based practice to make recommendations. Assists in developing resident plan of care to facilitate positive clinical outcomes and resident satisfaction.
PRINCIPLE DUTIES:
- Responsible for the data collection, analysis and monitoring of nursing quality indicators to present findings and recommendations to monthly QAPI Committee.
- Review newly admitted residents including a head to toe assessment, medication reconciliation, identify areas of concern and implement a plan of care for the front line team to follow.
- Conduct new admission assessments based on facility need and census dictates.
- Serve as case manager for HMO residents based on case load and facility need.
- Provide coordination of care with nursing and other resident services to ensure the continuity of the residents’ total regimen of care.
- Partner with the DON and DSD for clinical skills fair(s) annual competencies for licensed nursing staff, Registered Nurses in particular.
QUALIFICATIONS:
- Must possess current CA license as registered nurse, BSN preferred.
- Must have minimum of three years supervisory experience.
- Position requires individual to be detail oriented, organized with strong clinical and critical thinking skills.
- Ability to effectively communicate and present data and recommendations to interdisciplinary committee.
- Demonstrated competence with use of electronic medical record, PCC preferred.
- Ability to flex schedule based on facility needs.
CRITICAL FUNCTION:
- This position is essential to the safety of our residents and staff; and to the operation and function of the community. Employee may be recalled to report to work for an emergency, unusual occurrence, resident care need, visit from regulatory/governing agencies or any other event deemed necessary by the Executive Director, Administrator, and designee.
