We are a growing home health organization under new ownership and leadership, and we are looking for an exceptional Physical Therapist (PT) to join our team as we enter an exciting new chapter of growth.

What You'll Do

As a Home Health Physical Therapist, you will provide skilled physical therapy services to patients in the comfort of their homes while helping them improve mobility, strength, safety, and independence.

Responsibilities include:

  • Complete comprehensive physical therapy evaluations in patients' homes

  • Develop individualized plans of care based on each patient's needs and goals

  • Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence

  • Assess home safety and recommend appropriate equipment or modifications

  • Educate patients, families, and caregivers

  • Monitor patient progress and modify treatment plans as appropriate

  • Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team

  • Complete timely and accurate clinical documentation

  • Help prevent falls, hospitalizations, and avoidable declines in function

  • Participate in care coordination and discharge planning

What We're Looking For

  • Current Colorado Physical Therapist license or eligibility for Colorado licensure

  • Graduate of an accredited Physical Therapy program

  • Current driver's license and reliable transportation

  • Excellent communication and organizational skills

  • Ability to work independently and manage a patient caseload

  • Strong commitment to patient-centered care

  • Home health experience is preferred, but not required

  • Experience with geriatric or post-acute patients is a plus
