Job Title: Mobile Wound Care Nurse Practitioner (NP) or Physician Assistant (PA)
Location: Macomb County, Michigan
Schedule: Full-Time, Monday–Friday (5 days/week)

Job Summary:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile wound care team in Macomb County, MI. In this role, you will provide advanced wound care services to patients in skilled nursing facilities, assisted living facilities, and home-based settings. You will work autonomously in the field while being supported by a collaborative and experienced clinical and administrative team.
Key Responsibilities:

Perform comprehensive wound assessments, including staging, measuring, and photographing wounds

Develop and implement individualized wound care treatment plans

Perform debridement, dressing changes, and other wound care procedures as needed

Coordinate with facility staff, primary care providers, and other specialists to ensure continuity of care

Accurately document visits and maintain up-to-date patient records in the EMR system

Educate patients and caregivers on wound prevention and treatment strategies

Monitor progress and adjust treatment plans as clinically indicated

Participate in quality improvement initiatives and case reviews
Qualifications:

Active and unrestricted NP or PA license in the state of Michigan

National board certification (AANP, ANCC, NCCPA)

DEA license (or ability to obtain)

Prior experience in wound care, geriatric care, or post-acute care

Ability to work independently and manage a mobile patient schedule

Excellent clinical judgment, communication, and organizational skills

Valid driver’s license and reliable transportation
