JARC, located in Boca Raton, is a non-profit organization providing programs and services to educate and empower individuals with intellectual and developmental disabilities. We are looking for a Healthcare Coordinator to coordinate the services, entitlements, and medical needs of all residents. In this role, you will be responsible for reviewing medical records, working with all parents/guardians to receive and record all relevant medical records, current medication, history, medical providers and insurance information, and ongoing medical services.
Responsibilities:
- Promote the mission of JARC and conduct oneself in accordance with JARC core values.
- Work with Program Administrators/Directors to ensure compliance of all standards for all JARC Residential programs.
- Review residents' medication records to ensure compliance with state regulations governing medication administration. Group home reviews of these records will be done prior to monthly/annual monitoring for all JARC programs.
- Provide training, on the checking of documentation for medication administration, to ensure accuracy, and obtaining medical scripts according to APD/AHCA standards.
- Manage care and get updates on treatment and health status of any JARC resident receiving care in a hospital or rehabilitation facility.
- Attend all state agency and annual inspections at the group homes.
- Set up required initial central records and maintain all central records for JARC residents, as per licensure requirements for APD/AHCA.
- Coordinate resident participation in appointments and ancillary services. Accompany residents as required. Assure timely and appropriate communication of information.
- Participate in admissions meeting for all new potential clients.
- Participate in residents’ annual individual service plan meetings and quarterly reviews.
- Review distribution of all medication procedures with JARC staff as required.
- Conduct First Aid/CPR and other staff training as required.
- Responsible for supervising JARC staff in their coordination of medical care.
Qualifications:
- Bachelor's degree in human services field, or four years related experience. Registered Nurse preferred.
- Licenses and certificates as required by State Regulations. Certified instructor in CPR/First Aid.
- Must possess excellent management/leadership skills, strategic planning and organizational skills and effective verbal and written communication skills.
- Working knowledge of Microsoft Office required.
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures and governmental regulations. Write reports business correspondence, and procedure manuals. Ability to effectively present information, respond to questions from directors, public groups, and families.
