Full-Time

Position Overview
Ascend Home Health & Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our team and serve patients across Cache Valley and Box Elder County, Utah. In this role, you will provide high-quality, patient-centered care while coordinating services to support patients and their families throughout their healthcare journey. You'll play a key role in ensuring continuity of care, improving outcomes, and making a meaningful impact in the lives of those we serve.

Key Responsibilities

  • Provide comprehensive nursing care to patients in their homes and/or assisted living settings

  • Conduct initial and ongoing patient assessments and develop individualized care plans

  • Coordinate care with physicians, therapists, social workers, and other team members

  • Monitor patient progress and adjust care plans as needed

  • Educate patients and families on diagnoses, medications, and care plans

  • Complete accurate and timely documentation in accordance with regulatory standards

  • Participate in on-call rotation as needed

Qualifications

  • Current, active RN license in the state of Utah (or compact license if applicable)

  • Strong clinical assessment and critical thinking skills

  • Excellent communication and organizational abilities

  • Ability to work independently while collaborating with a team

  • Valid driver's license and reliable transportation
