MAJOR FUNCTIONS:

The Nurse Practitioner performs clinic-based medical evaluation and provides treatment with addiction medication for assigned caseload. The Nurse Practitioner is responsible for assuring that addiction medication services are coordinated with medical services delivered by other primary and specialty healthcare providers as well as providers of collateral services. The Nurse Practitioner is responsible for delivering a high level of quality care and adherence to all applicable Federal, State and Local laws and regulations governing delivery of substance use disorder treatment services.

SPECIFIC DUTIES & RESPONSIBILITIES:

1. Conducts medical assessment, diagnosis and treatment of opioid use disorders.

2. Documents medical histories and conducts physical examinations; orders and interprets clinical laboratory testing.

3. Provides direct patient care for prescribing and managing addiction medications.

4. Provides same day access to addiction medications when determined to be medically necessary.

5. Provides and monitors care in accordance with medical policies, procedures and protocols.

6. Ensures addiction medicine is integrated with psychosocial services.

7. Ensure addiction medicine is coordinated with services delivered by providers of collateral care.

8. Ensures medical records of enrolled patients are accurate and current.

9. Ensure all partial agonist and antagonist addiction medications are available according to medical necessity.

10. Ensures reporting of communicable diseases and infection in accordance with applicable law.

11. Ensures adherence to infection control policy and procedures.

12. Participates in development and implementation of QA and CQI activities.

13. Other duties as required.

REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES

1. Currently licensed as a Nurse Practitioner in the State of New York.

2. Current DEA#, Infection Control Certification.

3. Board Certified Preferred.

4. In good standing with New York State Department of Education Professional Licensing Boards and without any

outstanding malpractice judgments.

5. Advanced Nursing Degree from an accredited College or University.

6. Patient, supportive demeanor, able to work with challenging patient population
