Job DescriptionJob Title: Homelessness Case Manager

Location: Los Angeles, CA
Reports To: Program Manager

About the Role

The Homelessness Case Manager will provide intensive case management, housing navigation, and supportive services to individuals and families experiencing homelessness in Los Angeles. This role is designed for compassionate, detail-oriented professionals who can build trust, connect clients to community resources, and help them achieve long-term housing stability.

Key Responsibilities

  • Provide client-centered case management to a caseload of individuals experiencing homelessness.

  • Conduct comprehensive assessments (housing, health, mental health, benefits, employment, family reunification).

  • Develop and implement individualized service plans (ISPs) with clear housing and stability goals.

  • Assist clients with housing navigation, including landlord outreach, applications, and move-in coordination.

  • Connect clients to resources such as healthcare, behavioral health, legal aid, employment, and benefits enrollment.

  • Collaborate with community partners, service providers, and local agencies to maximize client support.

  • Maintain accurate and timely documentation in the Homeless Management Information System (HMIS).

  • Participate in case conferencing, multidisciplinary team meetings, and outreach efforts.

  • Monitor client progress and adjust service plans as needed.

  • Advocate for clients in accessing services and overcoming barriers to housing.

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field

  • Minimum 1 years of case management experience, preferably with unhoused or vulnerable populations.

  • Knowledge of Los Angeles homeless services system (CES, LAHSA, supportive housing programs) preferred.

  • Familiarity with trauma-informed care, harm reduction, and motivational interviewing approaches.

  • Strong organizational skills and ability to manage a high caseload.

  • Bilingual (English/Spanish) a plus.

  • Valid CA driver’s license and reliable transportation required.

Compensation & Benefits

  • Salary range: $23.00 - $26.00 per hour(commensurate with experience)
