Job Summary

Constant Care is seeking reliable and compassionate Certified Nursing Assistants (CNAs) to work as Clinic Escorts for nursing home residents in the Bronx.

The primary responsibility of the Clinic Escort is to accompany nursing home residents to medical appointments, clinics, hospitals, and other healthcare visits. The CNA Clinic Escort will ensure that residents are safe, comfortable, and properly supervised throughout the entire appointment process.

This is a great opportunity for a caring and dependable CNA who enjoys working with elderly residents and prefers a role focused on patient support, transportation, and medical appointments.

Responsibilities

• Escort nursing home residents to and from scheduled medical appointments.

• Accompany residents to doctors' offices, clinics, hospitals, and other healthcare facilities.

• Remain with residents throughout their appointments as required.

• Assist residents with wheelchairs, mobility devices, transfers, and positioning.

• Ensure residents remain safe, comfortable, and supervised while traveling and waiting for appointments.

• Bring necessary resident paperwork and appointment documentation.

• Return appointment paperwork and relevant information to the nursing staff after each visit.

• Communicate any concerns or changes in a resident's condition to the appropriate nursing staff.

• Maintain resident privacy and confidentiality at all times.

• Follow HIPAA requirements, facility policies, infection-control procedures, and safety guidelines.

• Maintain a professional and compassionate attitude when interacting with residents, families, nursing home staff, transportation personnel, and healthcare providers.

• Perform other resident-related duties as assigned.

Requirements

• Active New York State CNA certification is required.

• CNA certification must be current and in good standing.

• Previous nursing home, skilled nursing facility, rehabilitation, assisted living, or healthcare experience preferred.

• Experience working with elderly residents is preferred.

• Ability to safely assist residents with wheelchairs, transfers, and mobility needs.

• Must be dependable and punctual.

• Strong communication and interpersonal skills.

• Patient, compassionate, and professional.

• Ability to work independently and responsibly while accompanying residents outside of the facility.
