Position Summary:
The Case Manager provides and coordinates comprehensive client services within a short-term, live-in facility for homeless individuals. This facility serves a MICA (mentally ill, chemically dependent) population of approximately 200 adult males. The ideal candidate is proactive, takes initiative, and is experienced in working with clients facing complex behavioral health challenges.

Work Schedule: Sunday - Tuesday and Thursday (10am-6pm) and Wednesday 2pm-10pm. (Fridays and Saturdays Off)

Essential Duties & Responsibilities

  • Conduct intake process with new clients within 24 hours of admission, including explanation of resident rights, responsibilities, and grievance procedures.

  • Complete psychosocial, vocational/educational, and housing assessments with proper documentation.

  • Maintain accurate, complete, and timely records of client services, progress, and outcomes.

  • Develop initial service plans and conduct regular service plan reviews for an assigned caseload of 15–18 clients.

  • Provide comprehensive case management services, including coordination of housing, mental health, and substance use supports.

  • Prepare housing applications and assist clients throughout the housing application and interview process.

  • Facilitate individual and group counseling sessions, workshops, psychoeducation, and recreational activities.

  • Conduct room and dorm inspections to assess hygiene, cleanliness, and daily living skills.

  • Monitor and assess client progress or regression in behavior, appearance, and goal achievement.

  • Provide referrals for in-house and community-based medical, mental health, and social services.

  • Ensure clients attend meals and maintain appropriate nutritional intake.

  • Provide crisis intervention and respond effectively to emergency situations.

  • Assist in building evacuations during fire drills or emergencies.

  • Monitor medication compliance and collaborate with nursing staff as needed.

  • Facilitate discharge and transfer processes, including documentation.

  • Escort clients to housing interviews and appointments as necessary.

  • Attend staff meetings, case conferences, supervision sessions, and training as required.

  • Maintain current CPR/First Aid certification.

  • Follow all organizational policies, procedures, and applicable regulatory standards.

  • Flexibility to work some weekend and evening hours as required.

Required Education & Experience

  • High School Diploma or GED required; Bachelor's Degree in a Human Services field preferred.

  • Ability to work effectively with a culturally diverse population.

Preferred Education & Experience

  • One to two years of experience working with the homeless population preferred.

  • Experience in residential settings, group work, tenant organization, case management, referrals, and/or community outreach strongly preferred.

  • Background or familiarity with DHA, CARES, mental health services, and substance use disorder treatment is highly desirable.
