Schedule: Exempt, 16 hours per week | 8:00am - 4:30pm
What You’ll Do in This Role:
Home Health RN Case Managers assess patients’ needs, develop individualized care plans, coordinate with healthcare professionals, and ensure the delivery of high-quality skilled nursing care in the comfort of patients’ homes. This role calls for strong clinical expertise, excellent communication skills, and a compassionate, patient-centered approach to care. As part of a collaborative interdisciplinary team, you will support patients in achieving the best possible health outcomes while promoting independence and safety. This position follows a paid-per-visit compensation model.
Key Responsibilities
Patient Assessment
• Conduct comprehensive assessments of patients’ health status, including physical, psychological, and social factors.
• Evaluate home environments to identify potential risks and barriers to safe, effective care.
Care Planning
• Develop personalized care plans in collaboration with patients, families, and the interdisciplinary care team.
• Establish realistic, patient-centered goals based on individual needs and preferences.
Care Coordination
• Coordinate nursing care, therapy, and other healthcare services to ensure patients receive the support they need.
• Communicate effectively with physicians, therapists, social workers, and other providers to promote seamless care transitions and continuity.
Patient Education
• Educate patients and families on diagnoses, treatment plans, medications, and self-care strategies.
• Provide guidance on symptom management, complication prevention, and overall wellness.
Monitoring and Evaluation
• Regularly evaluate patient progress and adjust care plans to support optimal outcomes.
• Monitor vital signs, medication adherence, and overall well-being during home visits.
Documentation and Reporting
• Maintain accurate, timely patient records, including assessments, care plans, and progress notes.
• Document interventions, outcomes, and condition changes in accordance with regulatory standards and organizational policies.
Collaboration and Advocacy
• Partner with interdisciplinary teams to address complex needs and enhance care outcomes.
• Advocate for patients’ rights, preferences, and access to appropriate healthcare services.
Quality Improvement
• Participate in quality initiatives to strengthen home health services and maintain compliance with regulatory requirements.
• Identify opportunities for improvement and apply evidence-based practices to enhance patient safety and care delivery.
Skills, Knowledge & Expertise• Registered Nurse (RN) license to practice in the state of Michigan.
• Degree in Nursing, Bachelor's Degree in Nursing (BSN) preferred.
• Minimum of one (1) year of nursing experience.
• Previous experience in home health, case management, or related field preferred.
