Hospice Social Worker, MSW
Trellis Supportive Care is seeking a Part-time Hospice Social Worker (MSW) to provide support to patients and families for individualized hospice services at the Kate B. Reynolds Hospice Home in Winston Salem, NC.

Our staff has grown from a dozen individuals to over 300 specially trained professionals who work as a team to help patients live fully and comfortably, with hope — even during life’s most challenging time.
Schedule:
- Part-time - 20 hours per week
- Schedule options to work the 20 hours include: Saturday/Tuesday/Thursday OR Sunday/Wednesday/Friday
- Other schedule options can be discussed

Position Requirements: Master’s degree in Social Work required. Knowledge of the community’s social, financial, and health resources. Demonstrates excellent communication skills, both oral and written. Ability to work well within an interdisciplinary team setting as well as independently with minimum of supervision. Commitment to and understanding of the Hospice philosophy. Excellent organizational skills with the ability to handle a variety of tasks and to assign priorities to those tasks.
Position Duties and Responsibilities: Provide individualized Hospice services to patients and families in the Kate B. Reynolds Hospice Home. Assess patient/family emotional, social and financial needs and problems. Establish goals in the patient care plan to meet the psychosocial and emotional needs of the patient and family. Provide consultation, education, and support for other members of the Hospice team, as needed. Provide assistance to the patient and family in the utilization of the appropriate community resources and programs. Assist with Social Work on-call duties. Communicate and collaborate with the IDT regarding patient care issues. Attend IDT meetings as prepared to discuss the psycho-social and emotional needs of the patient and family and to collaborate with other team members. Assist with orientation of new staff, interns and volunteers. Explain fee for services and payment mechanisms, including the Hospice Medicare/Medicaid Benefits, for those eligible, to all new patients and families. Complete Financial Assessment when appropriate. Complete appropriate paperwork for discharges and revocations in a timely manner. Proceed with discharge planning from hospice home in a timely manner. Complete all Admission and PR visit paperwork completely and timely. Complete clinical documentation according to agency, state, federal and accreditation standards and discipline specific standards. Complete appropriate paperwork related to discharge planning, ie FL-2, PASARR, Medicare notices.
