Location:
This role is based in the Washington Navy Yard area, serving various zip codes across Washington, D.C. Contractors are assigned to specific zones within the district to best meet patient and staffing needs. Reliable transportation is essential, as all visits are performed using personal vehicles, with mileage reimbursed for travel between patient appointments.

Schedule/Hours:

• Monday to Friday
• 8:00 AM to 5:00 PM

Qualifications / Desired Experience:

• Valid Medical Doctor license
• BLS certification
• Minimum of 2 years of physical therapy experience, ideally in home health
• Proficiency in Homecare Homebase and OASIS documentation systems
• Reliable vehicle that meets all licensure and insurance requirements
• Willingness to float between zip codes as needed
• First-time travelers with OASIS experience are considered

Responsibilities / Duties:

• Conduct home physical therapy evaluations and treatments
• Complete daily documentation using Homecare Homebase/OASIS systems accurately and timely
• Coordinate care to support patient recovery and functional improvement
• Maintain compliance with all clinical and regulatory requirements
• Participate in team communications and coverage as needed to ensure patient care continuity
