Job Description
The Nurse Practitioner (NP) provides high-quality, evidence-based medical care to residents in a nursing home or long-term care setting. Working collaboratively with physicians, nursing staff, and other healthcare professionals, the NP ensures comprehensive assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions to promote resident health, function, and quality of life.
Key Responsibilities:

Conduct comprehensive health assessments, including history-taking, physical examinations, and review of medical records.

Diagnose and manage acute, chronic, and complex medical conditions in alignment with state and federal regulations.

Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team.

Prescribe medications, treatments, diagnostic tests, and therapies as permitted by licensure and scope of practice.

Provide ongoing evaluation of residents' conditions, adjusting care plans as needed to ensure optimal outcomes.

Deliver preventive care, health education, and counseling to residents, families, and caregivers.

Document all assessments, interventions, and care plans accurately and in a timely manner in the electronic medical record (EMR).

Participate in care plan meetings, quality assurance initiatives, and facility compliance activities.

Respond to urgent and emergent situations within the facility.

Collaborate with nursing staff to support clinical decision-making and mentor best practices in resident care.
Qualifications:

Master's or Doctorate degree in Nursing from an accredited Nurse Practitioner program.

Current Nurse Practitioner license and board certification (FNP, AGNP, or equivalent) in the practicing state.

DEA registration for prescribing medications (if required).

Minimum of 6 months NP experience; long-term care or geriatric experience preferred.

Excellent clinical assessment and critical thinking skills.

Strong interpersonal, communication, and team collaboration abilities.
Work Environment:

Direct resident interaction, interdisciplinary collaboration, and use of EMR systems.
