Job DescriptionDescription:

Joseph’s House is a residential respite care home in NW Washington, DC that serves primarily previously homeless men and women with advanced HIV disease. For most of these residents, some combination of mental illness, stigma, isolation, substance abuse, poverty, and/or domestic violence has prevented them from successfully managing their HIV disease. Once medically stabilized, many Josephs House residents transition to post-residential case management. These services can include medication management, housing, counseling, life skills training, advocacy, transportation, and community activities.


Job Summary:


The social worker at Joseph’s House is part of a multidisciplinary team which includes a registered nurse, director of clinical services, and staff. This team provides comprehensive intake, assessment, discharge planning, and case management services to all members of Joseph’s House Community. The social worker works directly with leadership, staff, residents, community friends, and community partners.


Duties/Responsibilities:

  • Build rapport and establish trusting, respectful relationships with clients and families
  • Evaluate prospective, new, and existing clients based on their needs, barriers, and goals
  • Assess strengths and needs of each community member
  • Develop an individualized discharge plan for every resident
  • Obtain identifying documentation (birth certificate, photo ID, Medicaid card, social security, etc.) for each community member
  • Develop and regularly update a Service Plan for every community member
  • Address client concerns and goals in real time using effective communication skills with the client and the nurse case management team
  • Identify appropriate community-based resources for community members (housing, counseling/treatment, employment) and advocate for these resources
  • Collaborate closely with Joseph’s House team, community partners, and resources
  • Document all service activities provided to community members for funding purposes
  • Supervision of volunteer SW aide and behavioral health specialist to assist community members in daily activities and ensure that these activities are documented for funding purposes
  • Maintain up-to-date files for each community member
  • Other tasks and responsibilities as required by residents and community needs.

Requirements:

Education and Experience:

  • Master’s degree required, PLUS two years post-Master's experience and attainment of a license
  • LISW or LICSW required
  • A minimum of 2 years working with vulnerable populations, including homelessness, substance use, mental illness, and HIV/AIDS

Other Requirements:

  • Ability to drive a vehicle
  • Valid and unrestricted driver’s license
  • Proof of vaccination against COVID-19
  • Local to the DC metro area.

Salary:

The salary range for this position is $73,000 - $78,000 annually, commensurate with skills and experience.

Schedule:

Monday - Friday with some flexibility based on residents' needs.
