Pharmacist - Home Infusion- On site
Location: Oklahoma City, OK
Position Summary
We are seeking a licensed Pharmacist to support home infusion services by ensuring safe, effective, and patient-centered pharmaceutical care. This role involves clinical review, compounding oversight, interdisciplinary collaboration, and direct patient and caregiver education to support high-quality outcomes in the home setting.
Key Responsibilities

Assess patient suitability for home infusion therapy

Design, monitor, and evaluate individualized pharmacy care plans

Recommend or adjust medication therapies based on clinical data and provider orders

Prepare, compound, label, and dispense medications in compliance with USP, DEA, and state regulations

Conduct nutritional assessments and provide education to patients and caregivers

Collaborate with nursing and interdisciplinary teams to ensure continuity of care

Monitor controlled substance inventory and promptly report discrepancies

Provide accurate drug information to healthcare professionals and patients

Participate in continuing education and professional competency programs

Adhere to company policies, quality assurance standards, and all regulatory requirements
Required Qualifications
-
Graduate of an accredited School of Pharmacy
-
Active Pharmacist license in the State of Oklahoma
-
One of the following:
-
1+ year of experience in sterile IV admixture procedures
-
Knowledge of non-sterile compounding, immunizations, and Medication Therapy Management (MTM)
-
Completion of an ASHP-accredited Pharmacy Residency Program
-
Completion of an Advanced Pharmacy Practice Experience (APPE)
-
Equivalent experience approved by pharmacy leadership

Proficiency in Microsoft Word, Excel, and PowerPoint

Strong customer service focus with a commitment to patient-centered care
Preferred Qualifications
-
Experience in home infusion, home care, hospital, or clean room environments
-
ASHP-accredited Pharmacy Residency completion
-
1+ year of indirect or supervisory experience
Schedule
-
Standard business hours with flexibility to support home infusion coordination
- Monday-Friday 9am-6pm or until close, no on-callnow but a possibility of on call every 7-8 weeks
