Lakeshore Talent is in search of a dedicated and compassionate Occupational Therapist (OT) to join a dynamic team within the healthcare industry!

As an Occupational Therapist, you will assess participant conditions, develop individualized treatment plans, and collaborate closely with interdisciplinary teams to integrate therapy into comprehensive Plans of Care. You will conduct home visits, educate participants and caregivers on safe and effective use of durable medical equipment, and actively contribute to participant progress through detailed documentation and reporting.

Location: Sacramenta, CA
Pay: $44 - 58/hr, depending on experience
Schedule: Monday-Friday 8 - 4:30
Travel: 10% - 25% Travel to participants homes and to cover at other facilities as needed. (Mileage is reimbursed and drive time is included in hours worked)

Key Responsibilities:

• Perform in home occupational therapy evaluations and treatment on acute, restorative, and maintenance levels
• Develop and implement treatment plans, educating participants, families, and staff to maximize safety and functional independence
• Maintain therapy department organization, equipment, and documentation in line with agency standards
• Participate in care planning, quality assurance, peer review, and utilization review activities
• Attend team meetings and provide thorough reports on participant progress

Required Qualifications:

• Graduate of an accredited Occupational Therapy program (Bachelor’s, Master’s, or Doctorate level)
• National Certification or eligibility for registration by the American Occupational Therapy Association (AOTA)
• State registration/licensure where practicing
• Valid driver’s license, personal transportation, and good driving record
• Minimum of 1 year experience working with frail or elderly populations

Preferred Qualifications:

• Two years of occupational therapy experience, ideally in a geriatric setting
