Schedule:

Position Summary &amp; Responsibilities:

Evaluate and treat SNF residents
</li>
<li>
Develop and implement individualized PT treatment plans
</li>
<li>
Provide gait training, therapeutic exercise, and mobility support
</li>
<li>
Document patient status, progress, and outcomes
</li>
<li>
Collaborate with interdisciplinary rehab team
</li>
<li>
Support functional improvement and safe discharge planning
</li>
