Position Summary
The Charge Nurse (RN) is responsible for providing clinical leadership and professional nursing care to assigned patients within a rehabilitation environment. Key Responsibilities
Patient Assessment
- Conducts timely and systematic assessments, including those at admission and during shifts.
- Coordinates admission processes while serving as charge nurse.
- Utilizes both licensed and unlicensed staff effectively for data collection.
- Incorporates feedback from patients and families, documenting accurately in the electronic health record.
- Evaluates and documents the educational needs of patients and families.
Care Planning
- Creates and updates individualized, outcome-oriented care plans.
- Supervises care planning for patients under the management of LPNs.
- Ensures care plans are aligned with the medical plan and include measurable objectives.
- Prioritizes care based on patient acuity and collaborates with the interdisciplinary team.
- Applies principles of adult and geriatric care, considering age-related factors.
Care Implementation
- Delivers safe, evidence-based nursing care in accordance with hospital policies.
- Performs and assists with clinical procedures as needed.
- Delegates tasks appropriately based on scope of practice and demonstrated competencies.
- Provides culturally sensitive, age-appropriate care that addresses both physical and psychosocial needs.
Evaluation of Care
- Assesses patient responses, progress towards goals, and the effectiveness of educational efforts.
- Modifies care plans as necessary.
- Evaluates discharge readiness and ongoing care requirements.
Communication & Documentation
- Encourages a positive, collaborative care atmosphere.
- Maintains confidentiality and ensures clear, timely communication.
- Documents care accurately and in accordance with organizational policies.
- Alerts physicians promptly regarding significant changes in patient conditions.
Qualifications
Education
- Graduate from an accredited nursing program (Associate Degree, Diploma, or BSN).
Licensure
- Possession of a current, unrestricted RN license in the state of employment and/or a compact license as applicable.
- Must fulfill all requirements set by the State Board of Nursing.
Certifications
- Required:
- Basic Life Support (BLS) certification through the American Heart Association (or obtained within 30 days of hire).
Physical Requirements
- Ability to push, pull, and/or lift up to 50 lbs regularly.
- Capability to stand and walk for extended periods throughout the shift.
- Ability to bend, grip, reach, kneel, twist, and squat to perform essential job functions.
