Position Summary

The Charge Nurse (RN) is responsible for providing clinical leadership and professional nursing care to assigned patients within a rehabilitation environment. This role encompasses thorough patient assessments, care planning, implementation, evaluation, and coordination of care, all while promoting interdisciplinary collaboration and achieving high-quality outcomes for adult and geriatric patients.

Key Responsibilities

Patient Assessment

  • Conducts timely and systematic assessments, including those at admission and during shifts.
  • Coordinates admission processes while serving as charge nurse.
  • Utilizes both licensed and unlicensed staff effectively for data collection.
  • Incorporates feedback from patients and families, documenting accurately in the electronic health record.
  • Evaluates and documents the educational needs of patients and families.

Care Planning

  • Creates and updates individualized, outcome-oriented care plans.
  • Supervises care planning for patients under the management of LPNs.
  • Ensures care plans are aligned with the medical plan and include measurable objectives.
  • Prioritizes care based on patient acuity and collaborates with the interdisciplinary team.
  • Applies principles of adult and geriatric care, considering age-related factors.

Care Implementation

  • Delivers safe, evidence-based nursing care in accordance with hospital policies.
  • Performs and assists with clinical procedures as needed.
  • Delegates tasks appropriately based on scope of practice and demonstrated competencies.
  • Provides culturally sensitive, age-appropriate care that addresses both physical and psychosocial needs.

Evaluation of Care

  • Assesses patient responses, progress towards goals, and the effectiveness of educational efforts.
  • Modifies care plans as necessary.
  • Evaluates discharge readiness and ongoing care requirements.

Communication & Documentation

  • Encourages a positive, collaborative care atmosphere.
  • Maintains confidentiality and ensures clear, timely communication.
  • Documents care accurately and in accordance with organizational policies.
  • Alerts physicians promptly regarding significant changes in patient conditions.

Qualifications

Education

  • Graduate from an accredited nursing program (Associate Degree, Diploma, or BSN).

Licensure

  • Possession of a current, unrestricted RN license in the state of employment and/or a compact license as applicable.
  • Must fulfill all requirements set by the State Board of Nursing.

Certifications

  • Required:
    • Basic Life Support (BLS) certification through the American Heart Association (or obtained within 30 days of hire).

Physical Requirements

  • Ability to push, pull, and/or lift up to 50 lbs regularly.
  • Capability to stand and walk for extended periods throughout the shift.
  • Ability to bend, grip, reach, kneel, twist, and squat to perform essential job functions.
