\n\nRN Qualifications\n

- Active Washington State Registered Nurse (RN) license\n
- At least 1 year of home health nursing experience in a Medicare-certified healthcare facility\n
- Proficient with HCHB (Homecare Homebase) EMR system\n
- Ability to work independently and travel to patient homes across King County\n
- Strong clinical assessment, communication, and organizational skills\n
\n\nHome Health Nurse Responsibilities\n

- Deliver skilled nursing care to patients in their homes throughout West Seattle and King County\n
- Create, implement, and evaluate individualized patient care plans\n
- Collaborate with interdisciplinary healthcare teams for comprehensive patient support\n
- Accurately document patient care using HCHB EMR\n
- Educate patients and families on disease management, medications, and self-care\n
- Maintain productivity standards (30 points for 40 hours)\n
