We are a Home Health Agency in Santa Clarita, CA that is looking for a self motivated, compassionate RN or LVN Home Health Intake Specialist!

The Intake Specialist will serve as the first point of contact for patients or customers, ensuring a seamless intake process. This role involves gathering and verifying essential information, coordinating services, and maintaining accurate records while providing excellent customer service.

Key Responsibilities:

1. Client/Patient Interaction:

   • Serve as the primary contact for individuals seeking services.
   • Conduct initial intake assessments via phone, in-person, or online.
   • Provide information about the organization’s services, policies, and procedures.

2. Documentation and Data Entry:

   • Accurately collect, verify, and input personal, medical, or service-related information into electronic systems.
   • Maintain confidentiality and comply with all HIPAA or industry-specific data security regulations.

3. Coordination of Services:

   • Schedule appointments, consultations, or service sessions based on availability and urgency.
   • Collaborate with internal teams and external partners to ensure proper service delivery.
   • Monitor the intake process and follow up on pending information or documents.

4. Customer Service and Support:

   • Address inquiries and resolve concerns in a timely and professional manner.
   • Build rapport and trust with clients to promote a positive experience.
   • Act as a liaison between patients and service providers.

Salary Range:
$41,600 - 52,000 including benefits.
