We're looking for a compassionate Per Diem Occupational Therapist to join our team and provide personalized, in-home care to the geriatric community in Duluth and the surrounding areas. Key Responsibilities:
- Assessment, planning, implementation, and evaluation of all occupational therapy modalities to patients.
- All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
- Improves or minimizes residual physical disabilities of the patient.
- Returns the individual to optimum and productive level within patients capabilities.
- Participates with all other home care personnel in patient care planning.
- Takes initial history and makes initial evaluation.
- Consults with physicians regarding patients progress
- Participates in quality assessment and performance improvement activities as assigned.
- Prepares clinical and progress notes.
- Helps develop the plan of care and revise, as necessary.
- Consults with family and agency personnel.
- Ability to do extensive bending, lifting and standing on a regular basis.
Requirements:
- A graduate of a OT Program recognized by the American Occupational Therapy Association or other National accepted Council or Association
- An active license to practice Occupational Therapy in MN
- A minimum of four (4) years experience as a Occupational Therapist, home care experience a plus
- Must have criminal background check
- Must have current CPR certification
- Must have active driver's license and clear driving record
- Computer skills.
- Ability to exercise initiative and independent judgment
