Position Overview
\n
Cognitive Institute, Inc. is seeking a compassionate and qualified Licensed Mental Health Professional (LMHP) to join our Mental Health Rehabilitation program. The LMHP provides clinical leadership and oversight while delivering and coordinating behavioral health services for individuals and families. This position is responsible for assessments, person-centered treatment planning, counseling, clinical documentation, crisis intervention, care coordination, staff supervision, and ongoing evaluation of services.

Key Responsibilities
\n
\n
Clinical Services & Treatment Planning
\n
• Conduct comprehensive mental health assessments and assist with clinical diagnosis and treatment planning.
\n
• Develop, implement, review, and monitor individualized, person-centered treatment plans.
\n
• Provide individual, family, and group counseling using appropriate evidence-based therapeutic approaches.
\n
• Ensure active client involvement in all aspects of treatment and care.
\n
• Participate in crisis intervention, risk assessment, safety planning, and continuity-of-care activities.
\n
• Complete Discharge Summary Reports and other required clinical documentation.
\n
\n
Care Coordination & Case Management
\n
• Provide oversight, assessment, and coordination of services for assigned recipients.
\n
• Coordinate and manage services provided through Cognitive Institute and other community-based providers.
\n
• Collaborate with physicians, nurses, clinical personnel, community agencies, and other providers to support treatment and continuity of care.
\n
• Assist clients with accessing appropriate community resources and services.
\n
• Participate in interdisciplinary and multidisciplinary approaches to assessment, treatment planning, and service delivery.
\n
\n
Clinical Supervision & Quality Assurance
\n
• Provide supervision and clinical leadership to mental health staff serving assigned recipients.
\n
• Complete required weekly evaluations for supervised new hires.
\n
• Complete monthly staff evaluations and annual competency evaluations as required.
\n
• Conduct monthly Quality Service Contact evaluations for assigned team members and follow up on identified discrepancies.
\n
• Review approximately 80% of assigned caseload charts quarterly to support documentation quality and regulatory compliance.
\n
• Provide leadership, guidance, and assistance to team members as needed.
\n
• Actively participate in agency evaluations and continuous quality improvement activities.
\n
\n
Documentation & Compliance
\n
• Document all services in accordance with clinical, agency, payer, and regulatory standards.
\n
• Enter progress notes into Clinical Advisor within 24 hours of service delivery with appropriate documentation and signatures.
\n
• Complete required progress-note processes on a weekly basis.
\n
• Complete and submit Concurrent Reports at least 10 days before existing Prior Authorizations expire.
\n
• Monitor the appropriate utilization of Prior Authorization units for assigned recipients.
\n
• Submit required personnel and administrative documentation to the business office.
\n
• Maintain compliance with Cognitive Institute policies, safety requirements, and applicable regulatory standards.
\n
\n
Professional & Organizational Responsibilities
\n
• Attend required staff meetings, clinical meetings, trainings, and agency activities.
\n
• Participate in meetings and trainings that support Cognitive Institute's programs and community involvement.
\n
• Respond promptly and professionally to telephone calls, emails, and other communications.
\n
• Maintain a professional and collaborative approach when working with clients, colleagues, and community partners.
\n
• Complete required annual training hours:

- 20 hours annually for full-time LMHPs
- 16 hours annually for part-time LMHPs
\n

\n
Minimum Qualifications
\n
Licensed Mental Health Professional (LMHP) as defined by the applicable Louisiana Behavioral Health Partnership (LBHP) requirements.
\n
\n
Candidates must maintain all applicable professional licenses, credentials, and qualifications required for the position.
\n
\n
