Case Manager II

Hiring Bonus of $1,000 to external candidates

We are seeking a Case Manager II to work on our Ramsey County Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. This caseload will serve clients mostly living in Ramsey County.

Hours: Monday-Friday flexible business hours

Location: Hybrid – in office and remote opportunities available

Hiring Range: $21.64 - $23.40
