Occupational Therapist Home Health Hours

  • M-F 8am-5pm (No on call or Weekends required)
  • Must live in or near Fayette and Nicholas counties

Occupational Therapist Home Health Compensation

  • The pay for this position is $62.00 an hour
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Occupational Therapist Home Health Responsibilities

  • Conduct comprehensive patient assessments, including physical, functional, environmental, and psychosocial needs to develop individualized plans of care.
  • Collaborate with patients and caregivers to establish realistic, measurable goals, including discharge planning.
  • Evaluate the patient’s home environment to ensure safety and suitability for effective care delivery.
  • Assess patient support systems and coordinate use of community resources to optimize outcomes.
  • Interpret and implement physician orders in accordance with occupational therapy standards and regulations.
  • Develop, initiate, and coordinate interdisciplinary plans of care, ensuring integration across all healthcare providers.
  • Identify patient care problems, establish therapy diagnoses, and modify interventions based on patient progress.
  • Perform ongoing re-evaluations and adjust treatment plans under physician direction as needed.
  • Provide patient and caregiver education on diagnosis, treatment plans, safety, infection control, and disease management.
  • Conduct comprehensive medication reviews and educate patients/caregivers on medication usage, side effects, and safety.
  • Maintain timely, accurate, and compliant documentation, including OASIS documentation and care plan updates.
  • Communicate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care.
  • Manage equipment and therapy supplies to support patient independence and functional improvement.
  • Supervise occupational therapy assistants and home health aides as required.
  • Monitor patient condition, recognize changes, and report concerns promptly per agency protocols.
  • Ensure strict adherence to infection control practices, universal precautions, and proper medical waste disposal.
  • Follow all agency policies, as well as state, federal, and payer regulations.
  • Participate in quality improvement initiatives (QAPI), staff meetings, and patient care conferences.
  • Maintain patient confidentiality and uphold patient rights at all times.
  • Utilize proper body mechanics and safety techniques to prevent injury to patients and self.

Occupational Therapist Home Health Requirements

  • Active Occupational Therapist (OT) license in the state of practice.
  • 3-6 months of experience preferred
