Mental Health Kokua (MHK) is hiring a Full-Time Case Manager in Kona to support adults with serious mental illness living independently in the community. This community-based role focuses on relationship-driven, recovery-oriented services that help individuals maintain housing stability, build life skills, and access essential supports.
In this role, you partner closely with individuals to coordinate care, navigate systems, and work toward personally meaningful recovery goals. This position is ideal for professionals who value collaboration, advocacy, and hands-on community work while making a lasting impact across Hawaiʻi Island.
What You’ll Do (Responsibilities)
- Care Coordination: Assess needs, develop service plans, and coordinate behavioral health, medical, and community services
- Client Support & Advocacy: Help individuals build daily living skills, access benefits, and navigate systems of care
- Housing Stability: Support housing retention and development of independent living skills
- Recovery-Oriented Support: Promote recovery using person-centered, trauma-informed practices
- Community Collaboration: Partner with providers, landlords, families, and community resources
- Crisis Support: Respond to client crises in accordance with MHK policies
- Documentation: Complete timely and accurate assessments, service plans, and progress notes
Case Manager Qualifications
- Preferred: Bachelor’s degree in psychology, sociology, social work, or a related human or social services field
- Experience: At least 1.5 years of experience in case management or working with individuals with serious mental illness and/or co-occurring disorders
- Skills: Strong communication, organization, advocacy, and relationship-building skills
- Certifications: TB clearance, CPR, and First Aid (or willingness to obtain)
- Fit for the Role: Ability to work independently in community settings while maintaining collaboration and accountability
Requirements & Work Conditions
- Valid Hawaiʻi driver’s license, current vehicle insurance, and safety inspection
- Ability to pass a criminal background check and alcohol/drug screening
- Ability to lift up to 25 lbs and travel throughout Hawaiʻi Island as needed
- Ability to work flexible schedules based on program and client needs
