Position Overview
Practice Model
● 100% outpatient primary care
● Nursing home rounds with support from APP's
● Full-spectrum Family or Internal Medicine (Pediatrics through Geriatrics)
● Strong need for chronic disease management and cardiac care
Schedule
● Monday-Friday, 8:00 AM - 5:00 PM
● Choice of 4-day or 5-day workweek
Clinical Support
● Average 15-18 patients per day
● Dedicated clinical support team (APPs, CMAs, LPNs, RNs)
● EPIC EMR
Compensation & Benefits
Guaranteed Compensation
● 300,000-325,000 base salary (guaranteed first 2 years)
● 150,000 sign-on bonus
● Up to 20,000 relocation assistance
● Partnership opportunity available
