Job DescriptionORGANIZATION SUMMARY

POSITION SUMMARY
We currently have an opening for a full-time Admissions Coordinator at our primary Portland location. The role of the admissions Coordinator is to support the Admissions Supervisor in coordinating admissions and to ensure that inquiries for treatment are reviewed and followed up on in a timely manner. The admissions Coordinator is generally the first-person patients contact when looking to engage in treatment and therefore must uphold a high standard of communication and attitude.

ESSENTIAL JOB DUTIES

  • Gather pertinent information from prospective patients, referents, and providers to ensure all required information is obtained.
  • Responsible for gathering and maintaining organization of incoming referrals and requested documentation.
  • Enter referral data into customer relation software.
  • Prioritize referrals for review for the clinical team.
  • Submit records requests for patients attempting to access Fora services.
  • Outreach to patients with outstanding items needed to complete referrals.
  • Coordinates admissions and monitors intake schedule.
  • Conduct screenings for potential patients.
  • Is familiar with treatment expectations, residential program structure and admissions process.
  • Work with clinical teams to make admissions decisions in a timely manner.
  • Answer calls and field questions regarding all Fora services and assist patients with accessing services at all levels of care within Fora.
  • Assist patients with accessing resources such as bus tickets, honored citizen applications and outside community resources.
  • Ensure customer inquiries receive a response in a timely manner.
  • Provides de-escalation and crisis mitigation tactics in person and over the phone.
  • Provides back up support for daily admissions tasks such as collecting urine drug screens, signing paperwork, and reviewing patient belongings.
  • Communicate and coordinate with staff from other departments effectively.
  • Maintain relationships with community partners.
  • Track data related to priority/high risk groups.


Professional and Ethical Responsibilities

  • Demonstrate ethical behaviors by adhering to established professional codes of ethics that define the professional context within which the employee works.
  • Adhere to applicable Federal and State laws and agency policies and procedures.
  • Recognize the importance of individual differences that influence client behavior.
  • Maintain (or complete) current addiction counselor certification or QMHP privileging.
  • Maintains healthy boundaries with clients and their families.


ADDITIONAL JOB RESPONSIBILITIES

  • Attend staff meetings as scheduled.
  • Additional training is to be completed as assigned in a timely manner.
  • Special projects and other duties assigned.
  • Participate in ongoing supervision and consultation.
  • Outreach to community partners.
  • Maintain quality services that are responsive to patient needs and reflect best practices.


SUPERVISORY FUNCTIONS
This position does not have supervisory duties.

QUALIFICATIONS

EDUCATION AND EXPERIENCE REQUIRED

  • High school diploma or equivalent
  • Experience in the behavioral health field strongly preferred


KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • Proficient in Microsoft Office Suite, specifically Outlook, Excel and Word
  • Experience with electronic health record and client management systems preferred.
  • Detail-oriented and organized
  • Demonstrated effectiveness in presenting ideas, transmitting information, or convey concepts to individuals or groups of people of varying educational, cultural and experiential levels.
  • Ability to maintain professional boundaries with patients, the community, and other staff members.
  • Ability to work collaboratively with patients, other treatment program staff, team members, and to treat everyone with respect and dignity at all times.
  • Written and organizational skills to complete all required documentation.
  • Communicates information effectively in writing and verbally
  • Strong organizational skills and attention to detail
  • Displays initiative and problem-solving skills
  • Marked ability to demonstrate and utilize self-awareness


OTHER REQUIREMENTS

  • For employees with a history of substance use disorder, Fora Health’s policy requires certification of a minimum of two years of sobriety for this position.
  • Ability to pass pre-employment or for cause drug tests.
  • Ability to pass DHS criminal background checks.
  • Position will require some amount of regular connection to work via cell phone to answer questions and respond to internal and external contacts.


WORKING CONDITIONS

  • Prolonged use of computers
  • Position generally works in an indoor office environment with infrequent travel between sites or to special events.
  • Position does not consistently require overtime. Adjustment of work schedule may be required.
  • Possible exposure to communicable diseases which can encompass a variety of illnesses and infections, including COVID, tuberculosis, Hepatitis A, B, and C, and HIV.
  • Possible exposure to upset, angry, severely traumatized, disabled or emotionally disturbed adults, adolescents, and families.
  • Exposure to potentially hazardous cleaning chemicals, personal protective equipment provided.
  • Fast paced, high volume work environment.
