Role Overview: Nurse Practitioner — PRN, Virginia Beach, VA
Legacy Care is seeking a skilled and compassionate Nurse Practitioner for PRN (as‑needed) coverage in Virginia Beach, VA. The successful clinician will provide primary and acute care to residents in post‑acute and long‑term care facilities across the Virginia Beach area on a flexible, on‑call basis. This PRN role requires strong clinical judgement, adaptability, excellent communication skills and a commitment to person‑centered, evidence‑based practice. Core duties include resident assessment, diagnosis and management, medication reconciliation, interdisciplinary collaboration and participation in quality improvement activities as scheduled.

Medical Malpractice Included
Key Responsibilities — Nurse Practitioner, Post‑Acute Care (PRN)
-
Conduct timely assessments and develop appropriate care plans for residents in post‑acute and long‑term care settings during assigned shifts
-
Diagnose, treat and manage acute and chronic medical conditions consistent with current evidence‑based practice
-
Perform medication management, including review of polypharmacy and coordination of medication reconciliation
-
Respond promptly to changes in clinical status, initiate appropriate interventions and coordinate transfers of care when necessary

Collaborate with nursing staff, physicians, therapists, social workers and facility leadership to deliver holistic, person‑centered care
-
Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences as scheduled

Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)

Support medication safety and stewardship initiatives, including antimicrobial and antipsychotic stewardship

Provide clinical consultation and education to facility staff and colleagues on complex clinical issues and best practices

Maintain professional standards of practice, including infection control, documentation and regulatory compliance
Essential Qualifications and Skills — Nurse Practitioner, Virginia Beach VA (PRN)

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution

Current and unrestricted Virginia Nurse Practitioner license and APRN certification

National certification as a Nurse Practitioner (for example family, adult‑gerontology) preferred

Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred

Proficiency with Electronic Medical Record systems and clinical documentation

Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams

Commitment to patient‑centered care, cultural sensitivity and ethical practice
-
Current Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) preferred where applicable

Ability to travel between facilities in the Virginia Beach area as required for PRN coverage
