Job DescriptionIntensive Case Manager - Onsite in Long Beach CA
LHH is hiring an Intensive Case Manager (ICMS) for our client in Long Beach CA. The person in this role will be responsible for client intake, assessments, and ongoing case management duties.

-Tuesday - Saturday, 730am-430pm or 830am-530pm
-Location: 100% onsite in Long Beach
-Hiring method: Contract to Hire / Temp to Perm
-Hourly pay rate: $24/hour
Responsibilities include,
- Performing initial intake and assessments with clients, including development of individualized service plans
- Conducting ongoing case management and follow ups with clients to determine individual goals and updating service plans
- Identifying and linking clients to all available resources (housing, food stamps, Medi-Cal, cash assistance, etc.)
- Performing documentation of case notes and adhering to compliance procedures
Qualified candidates will possess,
- 1 year or more of experience as an Intensive Case Manager (ICMS)
- Must be comfortable working with individuals with dual diagnoses (mental and/or behavioral health, substance abuse issues, alcohol abuse issues, etc.)
- Must be a compassionate individual, dedicating to helping others
Pay Details: $23.00 to $24.00 per hour
