Job Description

We’re looking for a compassionate, grounded, and dependable Social Worker to join our team at Spero Hospice. In this role, you’ll be helping patients and families navigate one of life’s most difficult moments with dignity and support.


You’ll work side-by-side with nurses, our medical director, and other care team members to make sure patients and their loved ones feel seen, supported, and connected — not just clinically, but emotionally and practically.


What you’ll do:

  • Meet with patients and families to assess needs and offer emotional support
  • Help build personalized care plans focused on comfort and quality of life
  • Guide families through hospice services and end-of-life decisions
  • Connect people with the right community resources and benefits
  • Be a steady, calming presence for patients and loved ones facing tough transitions

Compensation:

$70,000 - $85,000 yearly

Responsibilities:

  • Support patients and families emotionally throughout the hospice journey
  • Help people process tough emotions and cope with serious illness and grief
  • Complete psychosocial assessments to understand each family’s situation
  • Build care plans that reflect each patient’s needs, values, and goals
  • Connect families with helpful resources and community services
  • Work closely with nurses, doctors, and chaplains to provide well-rounded care
  • Guide families through hospice services and care decisions
  • Respect and honor cultural and spiritual preferences in all care
  • Offer grief and bereavement support after a patient passes

Qualifications:

  • Active NJ license (LSW or LCSW)
  • Experience in hospice, palliative care, or healthcare is a big plus
  • Solid understanding of end-of-life care and what families go through
  • Strong communication skills — clear, compassionate, and real
  • Able to build trust and offer support during emotional, sensitive moments
  • Familiar with local resources and services to help guide families
  • Comfortable working as part of a tight-knit care team
