JOB DESCRIPTION SUMMARY

The Inpatient Registered Nurse Case Manager plans, organizes and directs hospice services for the Inpatient clients. The CM provides hospice care and is skilled at assessing pain and other complex symptoms at the end of life. The professional nurse works with the interdisciplinary hospice team to plan and direct services to meet the needs of individuals and families in the General Inpatient setting. The Case Manager fully supports the Core Values and is accountable for upholding the Standards of Behavior of VNA Health.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

  1. Responsible for providing comprehensive case management for all patients- and coordinating care with Serenity House clinical staff.
  2. Assess and evaluates patient status by writing and reviewing care plans. Regularly re-evaluates patient and family needs. Revises the plan of care as necessary.
  3. Initiates the plan of care and makes necessary revisions as needed. The Plan of Care establishes goals, interventions, and includes the patient and family in the planning process.
  4. Uses health assessment data to assure diagnosis is coordinated with care plan.
  5. Provide nursing services, treatments, diagnostic and preventive procedures requiring the skills of a registered nurse according to physician orders and the in-patient facility protocols.
  6. Provides healthcare instructions to the patient/family as appropriate per assessment and plan of care.
  7. Assess changing patient conditions as needed and report changes to physician if indicated and coordinate care with nursing staff.
  8. Provides and maintains a safe environment for the patient and family.
  9. Coordinates care with other interdisciplinary team members and participates in IDT meetings and completes IDT notes on current patients.
  10. Completes weekly summaries reports on active patients as required- to include patient changes of condition, current medications and family/patient concerns.
  11. Assess and maintain patient safety.
  12. Assess and coordinate care of GIP patients by assuring the appropriate diagnosis, charting and care is provided as needed and also assure patient transitions off of GIP level of care when directed by physician.

  1. Attends and participates in clinical education, in-service education and nursing staff meetings.
  2. Participates in Quality Management and Utilization Review Activities.
  3. Maintain necessary records and reports in a timely manner.
  4. Applies working knowledge of state, federal local and accreditation regulations for the delivery of hospice services.

POSITION QUALIFICATIONS

  1. Current and active California RN license.
  2. Bachelor’s degree, preferred.
  3. Minimum one year acute care experience required; background in home health and/or hospice preferred.
  4. Current CPR certification.
  5. IV and Central Line experience desired.
  6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
  7. Basic working knowledge and skill of computers (email) required.
  8. Must demonstrate ability to communicate effectively, have initiative, dependability and good judgment.
  9. Must be able to collaboratively work as part of an interdisciplinary group, as well as independently and problem solve in a crises situation.
  10. Must be able to cope with family, emotional stress and be tolerant of individual lifestyles without judgment.
  11. Reliable transportation.

PHYSICAL REQUIREMENTS

  1. Ability to lift 50 lbs.
  2. Ability to push/pull 50 lbs.
  3. Ability to stoop, bend, and squat.
  4. Ability to walk up one flight of stairs.
  5. Ability to walk one mile.
  6. Ability to sit for one hour, minimum.
  7. Visual and audio acuity.
