Job Summary
In this entry-level case management role, you will be responsible for assisting individuals and families in accessing the health, social, and community-based services they need to thrive. You will serve as a trusted advocate, connecting clients to the appropriate programs and resources, while also providing ongoing support and guidance throughout their journey.
Key Responsibilities
- Conduct comprehensive assessments to identify clients' needs and develop personalized service plans
- Coordinate with a variety of community partners and service providers to facilitate access to essential services
- Maintain detailed case records and documentation to ensure efficient and effective service delivery
- Advocate on behalf of clients to ensure their needs are met and their rights are protected
- Provide emotional support and guidance to clients, empowering them to achieve their goals
- Participate in team meetings and collaborate with colleagues to identify and implement process improvements
Qualifications
- Bachelor's degree in social work, human services, or a related field
- Experience in case management or direct service delivery preferred
- Strong communication and interpersonal skills, with the ability to build rapport and trust with diverse clients
- Demonstrated problem-solving skills and a commitment to providing high-quality, client-centered care
- Knowledge of community resources and social services programs
- Proficiency in relevant software and data management tools
