Registered Nurse – Case Manager (Contract-to-Hire) | Savannah, GA

Location: Savannah, GA 31405
Shift: Monday – Friday, 8:30 AM – 5:00 PM
Pay: $50–$53/hr (Contract-to-Hire)
Mileage Reimbursement: $0.43/mile

About the Role:

We are seeking a skilled Registered Nurse Case Manager to join our team in Savannah, GA. This role focuses on comprehensive assessment and care coordination for high-risk members across Chatham, Liberty, Bryan, and Effingham counties. You will play a critical role in improving patient outcomes while ensuring cost-effective, individualized care plans.

Key Responsibilities:

• Perform comprehensive assessments of high-risk members.
• Collaborate with primary care providers to implement individualized care plans with targeted interventions.
• Continuously reassess and ensure appropriate levels of care.
• Arrange non-medical support (housing, transportation) to promote treatment compliance.
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources to achieve optimal patient outcomes.
• Maintain accurate and detailed records of clinical, functional, and fiscal outcomes.
• Identify opportunities for health promotion and illness prevention.
• Advocate for patients while maintaining privacy and confidentiality.

Qualifications:

• Education: Associate’s or Bachelor’s Degree in Nursing or related field.
• License: Current, unrestricted Georgia RN license.
• Experience/Skills:
• • Proficient with OASIS documentation and accuracy.
  • Knowledge and use of HCHB (Home Care Home Base).
  • Ability to analyze complex medical information and make rational decisions.
  • Case Management certification preferred.
• Productivity Requirement: 30 units per 40-hour week.
