Responsibilities

  • Provide high-quality patient care in a skilled nursing facility.

  • Assist in developing individualized care plans for patients.

  • Participate in ongoing professional development and training.

  • Engage in performance measurement and goal setting for personal growth.

  • Monitor and document patient progress and changes in condition.

  • Collaborate with healthcare team to ensure optimal patient outcomes.

  • Maintain accurate and timely documentation of patient care activities.

  • Educate patients and families on care procedures and health management.

  • Adhere to safety and infection control protocols at all times.

  • Utilize pipelines to assess and enhance patient care strategies.

  • Assist in training and mentoring new staff members.

  • Contribute to a positive team environment focused on achieving targets.


Experience/Qualifications

  • Experience in skilled nursing facilities or similar healthcare settings.

  • Strong understanding of patient care protocols and safety regulations.

  • Ability to develop and implement individualized care plans.

  • Commitment to ongoing professional development and learning.

  • Proven skills in monitoring and documenting patient progress.

  • Strong communication skills for patient and family education.

  • Ability to collaborate effectively within a healthcare team.

  • Experience in mentoring or training new staff members.

  • Knowledge of infection control protocols and safety measures.

  • Familiarity with performance measurement and goal-setting strategies.
