The RN Home Health Hospice Lead plays a vital role in the planning, coordination, and execution of Home Health and Hospice operations as assigned. This position acts as a clinical resource for caregivers within the departments and takes on delegated responsibilities from the department leader. Additionally, the lead supports departmental leaders in the oversight and development of policies, compliance, and key financial metrics.

Education: Must have graduated from an accredited nursing program.

Licensure: A current RN license in the state of practice is required. A valid driver's license is also necessary.

Preferred Education: A bachelor's degree in nursing (BSN) from an accredited institution is preferred.

Experience: A minimum of 2 years of experience in Home Health or Hospice is preferred.

Preferred Experience: Supervisory experience is advantageous.

Certifications: Must possess and maintain current CPR certification.

Additional Skills: Strong observational skills, effective nursing assessment, and communication abilities are essential. The candidate should be capable of providing timely written documentation, possess good time management and problem-solving skills, and be self-directed with minimal supervision. Flexibility and cooperation in fulfilling role obligations, along with the ability to work within an interdisciplinary team, are also important.
