Clinical Case Manager (Community-Based & Supported Housing - ASW, AMFT, APCC)


Location: SF, CA I Schedule: Mon–Fri, ~8am–5pm I Salary: $90K-$95K

About the Role

Join a new grant-funded program providing clinical support to adults living in seven transitional housing sites in and near the Tenderloin. This role blends clinical case management, care coordination, and onsite engagement—all with a trauma-informed, harm-reduction lens.

You’ll work directly with residents housed in the program, building trust, delivering clinical services, and partnering closely with onsite staff from a collaborating community organization. This position is ideal for an engaging, relationship-driven associate clinician who thrives in diverse, community-based work.


What You’ll Do


Clinical & Case Management

  • Provide individual and group therapy + clinical case management
  • Hold a caseload of ~30 residents
  • Conduct assessments, crisis intervention, and de-escalation
  • Deliver <20 clinical hours/week; the rest is case management & collaboration
  • Clinical supervision hours provided

Community Engagement

  • Float across 7 centrally located sites weekly
  • Build relationships with onsite case-management staff
  • Engage residents directly in their housing environment
  • Work as part of a new, collaborative, multidisciplinary initiative

Care Coordination & Documentation

  • Connect clients to medical, psychiatric, and social services
  • Maintain timely documentation and EMR records
  • Participate in meetings, training, and weekly supervision
  • Maintain CPR/First Aid (or obtain at hire); occasional on-call rotation


What We’re Looking For

  • Master’s Degree & Registered with BBS
  • ASW, AMFT, or APCC
  • Experience with homelessness, SUD, and/or mental health populations
  • Strong engagement and relationship-building skills
  • Comfortable working in high-need urban settings


Compensation & Benefits

  • Salary: $90K–$95K
