Clinical Case Manager (Community-Based & Supported Housing - ASW, AMFT, APCC)
Location: SF, CA I Schedule: Mon–Fri, ~8am–5pm I Salary: $90K-$95K

About the Role
Join a new grant-funded program providing clinical support to adults living in seven transitional housing sites in and near the Tenderloin. This role blends clinical case management, care coordination, and onsite engagement—all with a trauma-informed, harm-reduction lens.
You’ll work directly with residents housed in the program, building trust, delivering clinical services, and partnering closely with onsite staff from a collaborating community organization. This position is ideal for an engaging, relationship-driven associate clinician who thrives in diverse, community-based work.
What You’ll Do
Clinical & Case Management
- Provide individual and group therapy + clinical case management
- Hold a caseload of ~30 residents
- Conduct assessments, crisis intervention, and de-escalation
- Deliver <20 clinical hours/week; the rest is case management & collaboration
- Clinical supervision hours provided
Community Engagement
- Float across 7 centrally located sites weekly
- Build relationships with onsite case-management staff
- Engage residents directly in their housing environment
- Work as part of a new, collaborative, multidisciplinary initiative
Care Coordination & Documentation
- Connect clients to medical, psychiatric, and social services
- Maintain timely documentation and EMR records
- Participate in meetings, training, and weekly supervision
- Maintain CPR/First Aid (or obtain at hire); occasional on-call rotation
What We’re Looking For
- Master’s Degree & Registered with BBS
- ASW, AMFT, or APCC
- Experience with homelessness, SUD, and/or mental health populations
- Strong engagement and relationship-building skills
- Comfortable working in high-need urban settings
Compensation & Benefits
- Salary: $90K–$95K
