Position Summary:


The Home Health Clinician provides skilled healthcare services to patients in their homes in accordance with the physicians plan of care. The clinician coordinates care with the interdisciplinary team to support the patients recovery, independence, and overall well-being.

Key Responsibilities:


  • Deliver direct, professional clinical care to patients in their homes.

  • Develop and follow individualized care plans based on physician orders and patient assessments.

  • Perform regular assessments and document patient status, progress, and response to interventions.

  • Educate patients and families on disease processes, medications, safety precautions, and health maintenance.

  • Communicate and collaborate with physicians, care coordinators, and other members of the home health team.

  • Maintain accurate, timely, and complete clinical documentation in accordance with agency policies and regulations.

  • Monitor patient outcomes, report changes in condition, and recommend updates to the plan of care.

  • Adhere to infection control, HIPAA, and safety protocols during all patient interactions.

  • Participate in case conferences, staff meetings, and continuing education.

Role-Specific Duties:


Registered Nurse (RN):


  • Perform comprehensive nursing assessments.

  • Administer medications and treatments as prescribed.

  • Provide wound care, IV therapy, and chronic disease management.

Licensed Practical Nurse (LPN):


  • Assist RNs in implementing care plans.

  • Provide basic nursing care under supervision.

  • Monitor vital signs and report changes in condition.

Physical Therapist (PT):


  • Evaluate patients mobility and functional needs.

  • Develop and implement therapeutic exercise programs.

  • Educate on injury prevention and use of mobility aids.

Occupational Therapist (OT):


  • Help patients regain independence with daily activities.

  • Recommend adaptive equipment and modifications.

  • Provide training on fine motor skills and safety.

Speech-Language Pathologist (SLP):


  • Assess and treat speech, language, cognitive, and swallowing disorders.

  • Develop individualized speech therapy plans.

  • Educate families on communication strategies.

Medical Social Worker (MSW):


  • Assess psychosocial needs and provide counseling.

  • Connect patients with community resources.

  • Support end-of-life planning and advance directives.

Home Health Aide (HHA):


  • Assist patients with personal care (bathing, grooming, toileting).

  • Support basic household tasks and ambulation.

  • Monitor patient well-being and report concerns to supervisors.

Qualifications:


  • Valid state licensure or certification for applicable discipline.

  • CPR certification (as required).

  • Minimum of one year of experience (home health preferred).

  • Strong clinical assessment and communication skills.

  • Reliable transportation and willingness to travel to patient homes.

Working Conditions:
