DESCRIPTION

The Medical Case Manager is responsible for providing a single standard of care to patients. They demonstrate the knowledge and skills necessary to provide care appropriate to the age of their patients; demonstrate knowledge of growth and development; assess data reflective of the patient’s status; and identify each patient requirements relative to age specific needs along the continuum of care. Coordinates and manages an assigned case load to promote positive clinical focused outcomes. Reviews for the appropriate level of care, resource utilization and evaluates the need for services for continuity of care. Promotes safe and appropriate planning.

QUALIFICATIONS

  • High school diploma or GED from an accredited On-site institute
  • Knowledge of electronic medical records and other software like Microsoft

PREFERRED REQUIREMENTS

  • Current CPR/BLS Certification
  • Bilingual – English/Spanish or English/Creole
  • 2 + years of experience in a clinical setting
  • Basic computer knowledge, MS Word and MS Excel, internet, document with Electronic Health Records and/or authorization system with minimal typing/spelling errors, send e-faxes and email.
  • Patient Access experience

Any appropriate combination of relevant education, experience and/or certifications may be considered.

Education needs to come from an accredited school or educational college/institution.
