Submission Requirements
- 1 year Med Surg, Post Op, or Stepdown experience [required]
- Clinicians MUST be comfortable caring for up to 5-7 patients -- N:P ratio information encouraged on work history
- Surgical, post op surgical experience [required]
- BLS [required]

- At least 1 travel contract completed [preferred]
- State license [required]
- Not an employee of UnityPoint in the last 12 months [required]

· Copy of required certifications
· References - 1 supervisory reference required at time of submission [required]
**Must have taxable address outside of 50 miles from facility. **
Unless Madison where they will accept local with a 10% rate reduction.
