Are you ready to make a meaningful impact in the lives of patients? At Aveanna Healthcare, we are seeking a passionate Occupational Therapist (OT) to join our dedicated team. In this full-time role, you will provide clinical assessments, therapy interventions, and personalized treatment plans in a home health setting, all while collaborating with an interdisciplinary team to optimize patient outcomes.

What You'll Do:

As an Occupational Therapist, your responsibilities will include:

  • Conducting initial and ongoing assessments to evaluate mobility, coordination, strength, and overall function.
  • Administering, evaluating, and interpreting tests to set functional goals and outcomes.
  • Creating comprehensive written evaluations and care plans that define therapy needs and expected outcomes.
  • Delivering occupational therapy services in accordance with physician orders.
  • Maintaining and updating care plans through timely documentation and modifications as needed.
  • Leading discharge planning, including educating patients and caregivers on home exercise plans.
  • Coordinating services with healthcare teams, physicians, and family members.
  • Documenting care effectively to ensure compliance and minimize denials from payers.
  • Supervising other personnel, such as Occupational Therapy Assistants (OTA) and Home Health Aides (HHA).
  • Completing OASIS assessments as permitted by state regulations.

What You Need to Succeed:

Requirements:

  • Degree in Occupational Therapy from an accredited program
  • Current, unrestricted state license as an Occupational Therapist
  • Valid CPR certification

Preferred Qualifications:

  • Strong organizational and communication skills
  • Attention to detail and effective time management
  • Proven ability to build relationships with patients and team members
  • Experience in Home Health Therapy and familiarity with OASIS documentation
