Are you ready to make a meaningful impact in the lives of patients? At Aveanna Healthcare, we are seeking a passionate Occupational Therapist (OT) to join our dedicated team. In this full-time role, you will provide clinical assessments, therapy interventions, and personalized treatment plans in a home health setting, all while collaborating with an interdisciplinary team to optimize patient outcomes.
What You'll Do:
As an Occupational Therapist, your responsibilities will include:
- Conducting initial and ongoing assessments to evaluate mobility, coordination, strength, and overall function.
- Administering, evaluating, and interpreting tests to set functional goals and outcomes.
- Creating comprehensive written evaluations and care plans that define therapy needs and expected outcomes.
- Delivering occupational therapy services in accordance with physician orders.
- Maintaining and updating care plans through timely documentation and modifications as needed.
- Leading discharge planning, including educating patients and caregivers on home exercise plans.
- Coordinating services with healthcare teams, physicians, and family members.
- Documenting care effectively to ensure compliance and minimize denials from payers.
- Supervising other personnel, such as Occupational Therapy Assistants (OTA) and Home Health Aides (HHA).
- Completing OASIS assessments as permitted by state regulations.

What You Need to Succeed:
Requirements:
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license as an Occupational Therapist
- Valid CPR certification
Preferred Qualifications:
- Strong organizational and communication skills
- Attention to detail and effective time management
- Proven ability to build relationships with patients and team members
- Experience in Home Health Therapy and familiarity with OASIS documentation
