Job DescriptionDescription:

The Dietician is primarily responsible for ensuring optimal nutrition care for patients across the life span (perinatal. pediatrics, and adults). Assesses nutritional needs based on medical condition, age, and socioeconomic factors. Coordinates nutritional care through ongoing communications with the patient/client, their significant other and the interdisciplinary team. Educates patients/clients on long term nutritional care through individual discussions, group classes and community speaking

engagements. Serves as an educational resource to nursing and medical personnel. Responsible for carrying out the mission, vision, values and quality commitment of The Elizabeth Hospice.


The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.


Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.


Salary: $36.00 - $44.00 hourly. Subject to experience and qualifications.

Work Schedule: Candidate must provide at least 6 shifts of availability per month with at least 2 shifts on a weekend day.

On-call: 1-2 shifts per month

Work Location: San Diego County and Riverside County

Travel: Travel is required for patient visits.

Requirements:

Responsibilities:

• Performs nutritional assessments on assigned patients, involving review of patient charts and diagnoses. Demonstrates knowledge of age related nutritional requirements.

• Reviews, updates, and coordinates patient nutritional care and protocol implementation through ongoing communications with interdisciplinary team.

• Consults with case manager and patient concerning patient diets and food preferences, and instructs as needed.

• Documents all relevant aspects of nutritional care for patients in accordance with departmental standards.

• Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

• Adhering to the E-way standards is a requirement in performing my job duties.

Qualifications

Qualifications:

• Bachelor's Degree required, Master's preferred and 1-2 years related experience required, prefer 2-3 years; or equivalent combination of education and experience.

• Registration with American Dietetic Association required.

• Good oral and written communication skills.

• Current CPR certification.

• Current negative result for TB skin test.

• Must be able to use office equipment; telephone, copier, fax, computer

• Current California driver's license and proof of auto insurance.
