Key Responsibilities:
- Conduct thorough and accurate assessments, MDS, and care plans as assigned.
- Develop, initiate, and manage care plans and related activities.
- Prepare and distribute monthly care plan calendars promptly.
- Maintain and update all care plans and assessments to meet regulatory requirements.
- Monitor and audit clinical records, ensuring accuracy, timeliness, and compliance.
- Communicate persistent documentation issues to the Director of Nursing (DON).
- Safeguard the confidentiality of Resident and Facility information at all times.
Qualifications & Requirements:
- MUST HAVE PRIOR MDS 3.0 EXPERIENCE.
- Valid New Jersey RN License.
- Proven experience in Long-Term Care settings.
- Exceptional organizational skills with attention to detail.
- Professional, motivated, and compassionate with a strong work ethic.
- Solid computer skills and proficiency in relevant software.
- Excellent communication and interpersonal skills.
- Team-oriented with a positive and friendly attitude.
