Job Summary
The Patient Care Representative (PCR) supports patient communication regarding therapy progress, physician follow‑up, medication profile updates, and inventory needs for refills. The PCR also assists pharmacists by obtaining and documenting laboratory values.
Roles and Responsibilities
- Manage team schedules for patient follow‑ups, refill assessments, and deliveries; participate in daily huddles to coordinate workflow.
- Communicate with patients to assess inventory, refill orders, anticipate needs, report non‑compliance, and process orders with accurate labels and packing lists.
- Document all interactions and updates clearly and promptly in progress notes.
- Collaborate with patients, caregivers, clinicians, and pharmacists to discuss progress and escalate concerns.
- Coordinate with Warehouse staff to ensure adequate supplies and timely deliveries.
- Update medication profiles in the EMR for pharmacist review.
- Follow up with physician offices as allowed by scope of practice.
- Notify Reimbursement of hospitalizations, prescription changes, or patient status updates.
- Obtain and enter lab values into the EMR and forward them to prescribing physicians.
- Assist with scheduling equipment and medical‑waste pickups and provide necessary documentation.
- Perform additional duties as assigned by leadership.
- Maintain confidentiality of company and patient information, complying with HIPAA and all applicable laws.
Qualifications and Experience
Required:
- Pharmacy Technician training/certification per state requirements and an active, in‑good‑standing license/registration.
- Strong computer (Word, Excel, PowerPoint), communication, interpersonal, organizational, and problem‑solving skills.
- Ability to work under pressure, meet deadlines, and manage processes with strong attention to detail.
- Demonstrated performance and customer service excellence.
Preferred:
- Minimum 6 months of pharmacy technician or related experience; working knowledge of pharmacy procedures.
- Experience in Home Infusion, Specialty Infusion, or Durable Medical Equipment.
- 2+ years of data entry or related experience.

This is a Contract to Hire position based out of Knoxville, TN.
Pay and Benefits
The pay range for this position is $20.00 - $24.00/hr.
