Nurse Practitioners (NP) Needed ($140K-$160K/yr)

Workforce1 Healthcare Career Center is partnering with top NYC healthcare providers to connect qualified Nurse Practitioners (NP) to an immediate opening at a Clinic in Manhattan.

In this role, you will primarily manage our RPM, CCM, and Telehealth programs, performing initial patient evaluations, conducting follow-up visits, and developing personalized care plans. As part of a collaborative care team, you will work closely with case managers to implement and monitor these programs, ensuring optimal patient outcomes and adherence to treatment plans.

Core Responsibilities:

  • Oversee the day-to-day operations of the Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs, ensuring effective patient engagement and program compliance.
  • Perform comprehensive assessments for new patients entering the RPM or CCM programs, including medical history, current health status, and care needs.
  • Conduct regular follow-up visits, both virtually and in person as needed, to monitor patient progress, assess any changes in condition, and adjust treatment plans accordingly.
  • Create individualized care plans for patients, addressing specific chronic conditions and goals, and incorporating RPM data into the plan to track progress over time.
  • Work closely with case managers to ensure that patient care plans are followed, patients are engaged in their care, and any barriers to success are addressed. Participate in regular team meetings to discuss patient progress and make adjustments to care plans as needed.
  • Provide education to patients and families regarding disease management, RPM devices, and the importance of adherence to the care plan. Offer support and encouragement to patients in managing their chronic conditions effectively.
  • Maintain accurate and timely documentation of patient assessments, care plans, visits, and outcomes in the electronic health record (EHR). Ensure compliance with regulatory requirements and internal quality standards.
  • Continuously assess and improve the RPM and CCM programs to enhance patient outcomes and satisfaction. Identify opportunities for program enhancement and contribute to the development of best practices.

Qualifications:

  • Master’s degree in Nursing (MSN) with a Nurse Practitioner (NP) certification from an accredited program.
  • Active Nurse Practitioner license in New York with certification in Adult or Family practice preferred.
  • At least 2-3 years of experience in a clinical setting, with a focus on chronic disease management or telehealth preferred.
