The Admission nurse is a member of the interdisciplinary team who provides professional hospice care as prescribed by the physician in accordance with the policies and procedures of YoloCares. The Admission nurse renders care that is within acceptable professional practice standards in compliance with the medical plan of care and state and federal guidelines.
Responsibilities
- Obtains referral information from intake team.
- Meets with patients and families to explain the hospice program of care and obtains appropriate signed consents from the legal representative to begin hospice care.
- Completes an Initial Nursing Assessment and Plan of Care and submits to Clinical Supervisor.
- Evaluates the immediate need for supplies, equipment, medications and services and coordinates the provision of these services.
- Completes a written summary of the assessment and/or evaluation visit.
- Communicates family/patient and physician concerns to team and acts as consultant to resolve.
- Assumes responsibility for compliance with state, federal and local regulations in relation to admissions and practice.
- Accepts assigned duties in a cooperative manner.
- Attends monthly staff meetings, continuing education and in-service programs.
- Other duties as assigned by clinical supervisor
- Consistently meet all standards of conduct delineated in the Yolo Hospice Integrity Program: Standards of Conduct: A Guide to Compliance and Ethics.

Reports To: Chief Clinical Officer
Salary: $124,800 - $131,040 yearly
Schedule: Monday – Friday, 10:00 AM to 6:30 PM
