Job Details
- Conduct comprehensive patient assessments using nursing knowledge and tools
- Develop patient-centered care plans aligned with goals and values
- Identify risk levels and apply risk segmentation strategies
- Coordinate interventions and services across care settings
- Ensure smooth care transitions and continuity of care
- Communicate with insurance payers for care approvals and reimbursement
- Advocate for patients and participate in quality improvement efforts
Skills You Need
- Current RN license in North Carolina with at least 2 years of experience
- Strong knowledge of care coordination, planning, and transitions
- Excellent communication and collaboration skills in team settings
- Proficiency with electronic health records and related software
