About the Role

The Occupational Therapist (OT) in an acute inpatient rehabilitation setting evaluates and treats patients recovering from illness, injury, or surgery to improve functional independence in activities of daily living (ADLs), mobility, and cognitive/perceptual skills. The OT utilizes GG (Functional Independence Measure – FIM/GG) scoring to assess patient progress and document functional outcomes in compliance with CMS (Centers for Medicare & Medicaid Services) and other regulatory requirements.

POSITION’S ESSENTIAL RESPONSIBILITIES:

1. Patient Evaluation & Treatment Planning:

  • Perform comprehensive initial and ongoing rehab assessments in IPR, focusing on ADLs, mobility, cognition, and discharge readiness.
  • Administer GG scoring (Section GG) for IPR patients per CMS guidelines (e.g., GG0130: Eating, GG0170: Sitting to Standing).
  • Use standardized assessments (e.g., MoCA, MMT, FIM) and evidence-based interventions to maximize recovery.

2. Treatment Planning & Implementation:

  • High-intensity ADL retraining (dressing, bathing, toileting).
  • Neuromuscular re-education for stroke/TBI recovery.
  • Cognitive-perceptual rehab (memory, attention, executive function).
  • Home assessments & caregiver training pre-discharge.

3. GG Scoring & Documentation:

  • Accurately complete Section GG functional scoring at admission, discharge, and per IRF-PAI guidelines.
  • Ensure compliance with:
    1. CMS guidelines (Medicare Part A, IRF coverage criteria).
    2. Hospital policies (infection control, fall prevention).
    3. Joint Commission standards (safety, quality measures).

4. Patient & Caregiver Education:

  • Educate patients and families on home safety, fall prevention, and proper use of assistive devices.
  • Train caregivers in safe transfer techniques and mobility assistance.
  • Provide guidance on post-discharge exercises and adaptive strategies.

5. Interdisciplinary Collaboration:

  • Participate in weekly team meetings to discuss patient progress and discharge planning.
  • Communicate effectively with physicians, nurses, case managers/social workers, and other therapists.
  • Adjust treatment plans based on patient response and medical status.

6. Discharge Planning & Interdisciplinary Coordination:

  • Coordinate with case managers/social workers for safe community reintegration, health services or outpatient follow up
  • Educate patients/families on post-discharge care.
  • Ensure a smooth transition the next level of care.

7. Compliance & Professional Development:

  • Follow hospital protocols (e.g., isolation precautions, code response).
  • Maintain state licensure, NBCOT, BLS.

Qualifications:

  • Education: Master’s/Doctorate in OT (ACOTE-accredited).
  • Licensure: Current state OT license; NBCOT certification.
  • Experience:
    1. 1+ years of OT experience (preferred in inpatient rehab, acute care, or neuro rehab).
    2. New grads with strong clinical rotations in rehab settings may be considered.
    3. Familiarity with GG scoring preferred.
  • Skills:
    1. Knowledge of IRF-PAI & CMS regulations for Acute Inpatient Rehab.
    2. Adaptability to fast-paced, medically complex environments.
    3. Strong knowledge of neurological & orthopedic rehab.
    4. Ability to manage medically complex patients (e.g., post-stroke, spinal cord injury, amputees).
    5. Excellent communication and teamwork skills.
    6. Proficient in EMR documentation.
