Job DescriptionPhysical Therapist - Home Health | Full-Time PT Position in Lancaster

Key Responsibilities
- Conduct comprehensive physical therapy evaluations and assessments to determine patient functional limitations, mobility restrictions, and rehabilitation needs

- Deliver evidence-based physical therapy interventions including therapeutic exercises, manual therapy techniques, gait training, and pain management strategies
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate comprehensive patient care
- Provide patient and caregiver education on home exercise programs, fall prevention, assistive devices, and safe mobility techniques
- Monitor and document patient progress, outcomes, and response to treatment using electronic medical records
- Adjust and modify treatment plans based on patient progress and changing needs
- Ensure compliance with all federal, state, and agency regulations governing home health physical therapy services
- Manage caseload efficiently while maintaining flexible scheduling for home visits throughout the Lancaster service area
Required Qualifications
- Current, unrestricted Physical Therapist (PT) license in the state
- Graduate degree from an accredited physical therapy program (DPT, MPT, or equivalent)
- Valid driver's license and reliable transportation for home visits
- Current CPR/BLS certification
- Strong clinical assessment and critical thinking skills
- Excellent verbal and written communication abilities
- Exceptional interpersonal skills with a patient-focused, compassionate approach to care
- Strong organizational and time-management capabilities
- Proficiency with electronic documentation systems and healthcare technology
- Ability to work independently while being part of a collaborative healthcare team
Preferred Qualifications
- Previous home health physical therapy experience
- Geriatric rehabilitation experience
- OASIS documentation experience
- Specialty certifications in orthopedics, geriatrics, or neurological rehabilitation
