For nearly 13 years,Trivida Functional Medicine has helped clients heal and thrive through deeply personalized, root-cause care. Now, we’re expanding that mission through Lineage Wellness, our new membership-based Direct Primary Care (DPC) practice built to bring time, trust, and partnership back to medicine.
This DPC model allows us to offer longer visits, timely access, and true connection, free from the limitations of insurance. Our patients are lifelong learners of their own health, and we are here to guide them through every stage of their wellness story.
Position: Direct Primary Care Provider (Part-Time to Full-Time Growth)
We are seeking a compassionate, functionally informed Family Nurse Practitioner or Physician Assistant to join our growing DPC practice. This is a part-time, in-person position that will expand to full-time as patient volume increases (we expect about 8-12 months depending on projected growth).
The ideal candidate has a strong foundation in primary care, is comfortable managing patients across the lifespan (preferably teens through seniors), and brings curiosity and confidence in hormone optimization, thyroid management, and comprehensive lab interpretation.

Responsibilities:
Primary Care and Clinical Management
- Provide comprehensive primary care for patients of all ages, including physicals, chronic disease management, and acute visits.
- Conduct thorough evaluations and develop care plans that integrate conventional and functional medicine approaches.
- Manage men’s and women’s hormone therapy, thyroid treatment, and medication for mental and emotional health.
- Order and interpret advanced labs, imaging, and diagnostics.
Patient Education and Relationship Building:
- Take the time to listen deeply, educate, and empower patients to take ownership of their health.
- Guide patients through lifestyle and preventative interventions.
- Serve as a trusted partner in long-term health and wellness.
Collaboration and Growth:
- Work closely with the Trivida Functional Medicine team to ensure coordinated, comprehensive care.
- Participate in developing clinic systems, memberships, and processes as the practice grows.
- Stay current with evolving research in both conventional and integrative medicine.
Qualifications
Education and Experience:
- Licensed NP, PA in good standing.
- Minimum 2 years of primary care experience preferred.
- Interest in functional and integrative medicine with a desire to continue learning.
- Competent and confident in hormone therapy, thyroid management, and lifestyle-based interventions.
Skills and Attributes:
- Warm, curious, and grounded in patient-centered care.
- Skilled communicator and educator.
- Detail-oriented with strong follow-up habits.
- Open-minded and eager to grow within an evolving practice.
Compensation and Benefits:
- Hourly Rate: $60–$70 per hour (transitioning annual salary as role scales to full time).
- Bonuses: Performance-based bonuses tied to patient retention and practice growth.
- PTO and benefits negotiated for Part time.
- Opportunity for long-term growth and vested potential within two years.
- No insurance billing, quotas, or production-based pressure.
- Systems, operations, and team management already in place so you can focus on patient care.
Schedule and Location:
- Starting part-time (24–28 hours per week), scaling to full-time as patient capacity grows Likely around the 8-12 month mark.
- In-person role based in Denver Tech Center / Centennial, with virtual visits for patient flexibility.
- Clinic hours: TBD, but generally Monday to Friday, 9 a.m. to 5 p.m.
