We are currently seeking a full-time Hospice Case Manager (RN) to join our growing interdisciplinary team.

Key Responsibilities
- Manage and coordinate hospice care for an assigned patient caseload
- Perform comprehensive assessments and develop individualized plans of care
- Collaborate closely with physicians, social workers, aides, chaplains, and other team members
- Provide symptom management, medication oversight, and patient/family education
- Ensure accurate, timely clinical documentation and regulatory compliance
- Participate in interdisciplinary team meetings and on-call rotation as scheduled
Qualifications
- Current Utah RN license in good standing
- Hospice or home health experience strongly preferred
- Strong clinical judgment, organization, and communication skills
- Ability to work independently while functioning as part of a collaborative team
- Compassionate, patient-centered approach to care
- Reliable transportation and willingness to travel to patient homes
